To proceed with an appointment, screening is required. Please include the following in your inquiry:

•  Photo of Driver's License /ID (sent via email)

• Employment Verification LinkedIn or Email from work email

• Two Recent Provider References and their websites/handles (within the last 12 months)  

                      Please note - providing references alone is considered incomplete  screening.

Privacy and safety are paramount! All information shared is secure and discarded upon completion of screening.
